How do I enroll for my new hire benefits?

  • During your finger printing appointment, you were scheduled to attend a mandatory New Hire Induction / Benefits Meeting.  You were also given a Benefits packet which included your scheduled meeting date and time.
  • After attending your New Hire Induction / Benefits Meeting, you must complete the following steps within 31 days of your hire date.
  • To view a complete list of new hire benefit documents that must be submitted to your Benefits Specialist, please click on the Required Benefits Documents Link located on the right hand side of this web page.

Department of Community Health, State Health Benefit Plan

  • If you wish to enroll for the first time or transfer existing State Health Benefit Plan, health insurance, you must complete the required Membership Enrollment Form within 31 days of your hire date and return it to the Benefits Specialist at the Central Office.
  • If you wish to decline health insurance as a new hire, you must complete the Declination Form within 31 days of your hire date and return it to the Benefits Specialist at the Central Office.

Shaw Hankins (Other Insurance Options)

  • You must go online within 31 days of your hire date, even if you wish to decline every thing in order to assign beneficiaries to your Board Paid Basic Life & Accidental Death & Dismemberment Insurance.
  • You will need to be sure to have your Bluebook ShawHankins User Guide with you to begin the enrollment process.
  • Go to www.douglascounty.bswift.com.  (Make sure your pop-up blocker is turned off.)  Enter your user id and password as instructed in your blue book and complete your enrollment process.
  • You must click the "DONE" button at the end in order to complete your enrollment process.  Once you click the "DONE" button, you will be prompted to print your Confirmation Form. 
  • Print two copies of your Confirmation Form.  Keep one for your records and send one copy to your Benefits Specialist at the Central Office.

Allstate Cancer Insurance New HireEnrollment Information

  • If you are interested in enrolling into the Allstate Cancer Insurance, you must complete the Cancer Interest Form.  Please return this form directly to Shaw Hankins.
