All new student registration will be handled at the Central
Registration Center located in the Gym behind the Annex
Registration Center: 4841 Highway 5 Douglasville, GA 30135

Directions:
Take Highway 5 south for five miles from intersection with Interstate 20
Pass entrance to Bill Arp Park and CO Annex building in old school building
Turn right immediately past school building into driveway (look for signage)
Park in indicated area and use only the gym door for the Registration Center
Parents should verify that they have all documents listed below with them before going to the Registration Center.
Students must attend the school located in the district in which they live. Call 770-651-2400 to determine the designated school. Do not depend on any source, other than the school system, to find out which school a child will attend. NOTE: Presenting falsified information is punishable under penalty of perjury.
All of the following information/documents must be provided to enroll your student(s) in the Douglas County School System:
Bringing children to the Registration Center is not necessary or encouraged.
Use this checklist to verify that documents are in your possession before going to the Registration Center:
Birth Certificate: Certified Copy
Georgia Immunization Form 3231
Georgia Ear, Eye & Dental Form 3300
Student's Social Security Card or Waiver
Parent/Guardian Photo ID
Custody/Guardianship Papers (if applicable)
Withdrawal Form AND Last Report Card
Middle & High School: Must Bring Student's Discipline History
High School: Must Bring Student's Transcript
Douglas County Vehicle Tag Receipt
Proof of Residency as required below:
If you rent: You must bring your CURRENT lease agreement and your CURRENT Utility Bill or Connection Receipt. Your Utility bill must be no more than 30 days old and not indicate disconnection or pending disconnection.
If you own: You must bring your CURRENT Mortgage Statement or Tax Bill and your Utility or Connection Receipt. Your Utility bill must be no more than 30 days old and not indicate disconnection or pending disconnection.
Multi-Family Residence Form: If living with family or friends, form must be signed by owner or leasee and notarized. Their proof of residency must be provided as indicated above.

Completed forms required for registration: These forms must be completed for registration. Please print and complete these forms before registering student(s). Each form must be completed for each student enrolling.

NOTE: Look at your records again. Be sure you secured all documents before going to the Registration Center. It will save you multiple trips as registration without full documentation is not allowed. Presenting falsified information is punishable under penalty of perjury.
